Sports Injury Insurance

If you were injured playing sports, you may be covered. We've compiled a list of some sports organizations that have coverage for Athletic Therapy as well as a link to the form to fill out.

Hockey Canada

Hockey Canada's Insurance covers 100% of expenses not covered by personal insurance plans to certain limits. This also Includes any teams that play under the Hockey Canada umbrella, including all minor hockey teams.  The claims form on the right must be completed by the athlete and the association,  then a referral from a physician filled on page two of the claims form specifying the need for treatment from an Athletic Therapist. Alberta Gymnastics Federation

All athletes registered under the Alberta Gymnastics Federation are covered up to $1500 per injury when the form on the right is filled out and with a referral from a physician specifying the need for treatment from an Athletic Therapist.

Rugby Alberta

All athletes and teams who play under Rugby Alberta are covered up to $300 per year when the form on the right is filled out and with a referral from a physician specifying the need for treatment from an Athletic Therapist.

Alberta Soccer Association

All athletes and teams who play under the Alberta Soccer Association are covered up to $500 per injury when the form on the right is filled out and with a referral from a physician specifying the need for treatment from an Athletic Therapist. Injury Claims Form

Volleyball Alberta

 Volleyball Alberta has coverage for injuries occurring during sanctioned practices and games including those occurring with your club team.  Pages 13 and 14 of the document on the right are the claims form which then need to be accompanied by a Physicians Referral specifying the need for treatment from an Athletic Therapist and $50/appointment to a maximum of $500 will be covered. Athletes competing for the following national and provincial teams are covered. These include, Athletics Canada, Biathalon Canada and Alberta, Bobsleigh Canada, Canadian Sport Institute, Canadian Sport Centre Atlantic, CanoeKayak Canada, Cross Country Ski Canada, Fencing Canada, Gymnastics Canada, Institute National du Sport du Quebec, Luge Canada, Rowing Canada, Paladins RMCC, Sailing Canada, Soccer Canada, Softball Canada, Ski Jumping Canada, Squash Canada, Swimming Canada, Synchro Canada, Tennis Canada, Triathlon British Columbia, Volleyball Canada, Water Ski and Wake board Canada, and Wrestling Canada Lutte.
